Interscholastic Athletic Participation Form (Green Form)
Student athletes are responsible for having this form completed by a Parent/Guardian and the Health Office
PRIOR to trying out for any sport. Completed forms should be returned to your coach on the 1st day of

Use of Private Vehicles for Athletic Transportation
Student athletes should ride the bus with the team to and from games. If there is a special
circumstance preventing an athlete from taking the bus, he or she is only allowed to travel with
his or her parent / guardian.

If it becomes necessary for members of your team to use private vehicles for transportation to any events, please have their parents/guardians fill out on of the attached forms and return to you. This must be done before they will be allowed to drive/ride in private vehicles.
