Work Permit Process and Form
All students 14 years and older must visit http://www.mass.gov/dos/youth/
This state website contains all of the rules and a downloadable application for a work permit. Once this work permit is downloaded, read, and filled out please return to the R.J. Grey Counseling Office for processing for your permit. Hard copies of this application are available at the Counseling Office. Once submitted, the process may take a few days.
Steps to follow:
- Read the info on the website http://www.mass.gov/dos/youth/
- Download the Work Permit form from the website.
- Obtain a job offer from a potential employer.
- Ask employer to fill out their part of the Work Permit form.
- Ask your physician to fill out a certificate of health. (Our school nurse may also do this IF you have a current physical on file.)
- Obtain parent signature for approval of employment options.
- Return completed form along with proof of age (birth certificate or passport or immigration record) to the R.J Grey Counseling Office for processing.
- Wait to receive your official Work Permit and Identification Number.
** If you are applying for a work permit in the summer time, please follow all of the steps above BUT submit completed form to the high school Counseling Office.
